Who We Are Our Philosophy Contact OnPointe

The where and what of the care we provide:
Our people, Our processes, Our protocols

OnPointe is a family of companies that provides patient centered healthcare services. These services are available through a variety of settings including hospital based skilled nursing units, transitional care skilled nursing facilities, long-term care nursing facilities, assisted living facilities, and patients’ own homes.

The founders and predecessor companies of OnPointe have maintained a family tradition of caring for seniors that spans nearly 100 years, its history extending from the sanitariums of the early days to the patient centered array of facilities and services offered today.

In each of its markets, OnPointe works with a variety of partners, both with and without financial affiliation, to form a network of healthcare options.

Patients are guided to the appropriate setting with care coordination services that facilitate moving through multiple sites of care, as dictated by the patient’s medical condition and financial realities. OnPointe offers care transition services to all providers involved in the patient’s care episode, regardless of ownership or financial affiliation. OnPointe believes this type of “open architecture” network is required to provide true patient-centered care.

We strive to assemble best-in-class options in both clinical services and physical facilities. In many cases, OnPointe has built or is building new facilities in order to provide top-level options.

Due to its extensive experience in both development and operations across the senior care continuum, OnPointe can design a solution that is perfectly customized to each market.  From its breathtaking facility designs to its creative, person-centered service delivery packages to its ground-breaking partnership models in the local healthcare community, OnPointe carefully chooses each element that makes up the overall project in order to maximize stakeholder experience.
